HOW DO I.... Patients can register at the surgery by completing the back of their medical card or with a registration form, which is available from reception. All patients over the age of five will be offered an appointment with the practice nurse for a health check. Patients will also be given a health questionnaire when requesting to join the surgery and this, along with the registration form or medical card, must be completed and returned to the practice nurse at the health check before the patient can be registered with us. Patients have the right to express a preference to see a doctor of their choice. Whilst we will endeavour to comply, there may be times when this is not possible and, if this were the case, an explanation would be offered. |
