HOW DO I....
OBTAIN A REPEAT PRESCRIPTION?
We run a computer-based
service for repeat prescriptions. Requests can be handed in to the surgery, posted
or faxed. Please tick the items you require on the slip provided and place it
into the box in the entrance, or give it to the receptionist well before your
medication is due to run out. If you are on more than one type of medication,
please try to order the items you require at one go, rather than one item one
week, then another the next week etc.
Repeat prescriptions will normally
be ready for collection within 48 hours, but please note the information on the
slip suggesting when medication will need reviewing. Prescriptions can be posted
if you include an SAE. We will not take prescription requests over the telephone
as mistakes are more likely to occur.
There is a pharmacy rota in the local
paper or on display in the local chemist shop.
HOW DO I....
OBTAIN A SICK CERTIFICATE?
For all illnesses lasting six working days or less,
a sick note from you doctor is not required. Instead, on returning to work complete
a self-certification form which is available from your employer or from the surgery.
If a note for a private insurance scheme is required, a charge will be
made, as this service is not covered by the NHS.
